The Plague are the remnants of Vault 59, yet another Vault screwed over by a decaying water chip and a faulty Vault Door (this one could only be opened from the outside). The story certainly sounds like it could be in a Fallout game (SPOILERS AHEAD): Alton, IL was one of the safer cities in post-war USA until The Plague arrived.
There was obvious care put into designing this, and despite me never visiting Alton personally, I certainly get the feeling that this was made by someone who comes from the city and has a personal connection to it. The interiors and exteriors are all unique from each other and the locations in DC. After an intro that is immediately forgotten as soon as your plane crashes, you are dropped in a surprisingly large, nicely detailed, and impressively varied landscape that feels like its own town and not just an extension of DC.
If you want to see what a competent, DLC-sized mod looks like, “Alton, IL” is a good place to start for Fallout 3 players.
